Phenobarbitone (Phenobarbital) Blood Level Test for Dogs & Cats

The Phenobarbitone (Phenobarbital) blood level test measures the concentration of phenobarbital in your pet’s serum. Phenobarbitone is the most commonly used antiepileptic medication in dogs and cats, prescribed to control seizures. Monitoring serum phenobarbitone levels is essential to ensure your pet receives a therapeutic dose that effectively controls seizures while minimizing the risk of toxicity and side effects, particularly liver damage.

Why Is This Test Important?

Phenobarbitone has a narrow therapeutic window. Too low a concentration fails to control seizures, while too high a concentration can cause severe side effects including sedation, ataxia, increased thirst and urination, and most importantly, hepatotoxicity (liver damage). Regular monitoring of serum levels is critical for pets on long-term phenobarbitone therapy. In dogs, therapeutic serum concentrations are generally considered to be between 20–40 μg/mL, while in cats, therapeutic concentrations are typically 15–45 μg/mL.

When Is This Test Recommended?

• Pets newly started on phenobarbitone therapy (initial level check 2–4 weeks after starting)
• Pets experiencing breakthrough seizures despite treatment
• Dose adjustment evaluation
• Routine monitoring every 6–12 months for pets on stable therapy
• Suspected phenobarbitone toxicity (sedation, wobbliness, jaundice)
• Pets with suspected liver disease on phenobarbitone

What Does the Test Involve?

A blood sample is collected at a consistent time relative to the last dose of medication (ideally just before the next scheduled dose, known as a trough level). The sample is sent to a laboratory for precise measurement of serum phenobarbitone concentration.

Understanding Your Results

Results are interpreted alongside your pet’s seizure control, clinical signs, and liver function tests. If levels are subtherapeutic, your veterinarian may recommend a dose increase. If levels are supratherapeutic or liver values are elevated, a dose reduction or drug switch may be considered.
